DETAILS OF REQUESTS MADE TO THE DEPARTMENT OF HEALTH AND HUMAN SERVICES
UNDER THE FREEDOM OF INFORMATION ACT – ABOUT THE OUSE DISTRICT HOSPITAL

 The details of information sought under the Freedom of Information Act are as below.

1.       All and any drafts of documents, notes, memoranda, emails, correspondence and the like from January 2007 to date – relating to plans or actions concerning existing and future services proposed to be implemented at, or removed from, the Ouse District Hospital.

 

2.       Any and all letters, notes, memoranda, emails, correspondence and the like between the Tasmanian Government and the Australian Government from January 2007 to date – relating to residential aged care beds, licences, status, transfer of licences and the like – with respect to Australian Government funded beds at the Ouse District Hospital.

 

3.       Any and all drafts, memoranda, notes and the like – relating to a letter authored by P. Leedham, director and titled “Open Letter to Family and Residents of Ouse District Hospital” dated 24 May 2007.

 

4.       Copies of budget papers for Ouse District Hospital from July 2004 to date – including audited annual statements and including breakdowns of personnel employed.

 

5.       Monthly hospital returns for Ouse District hospital from July 2004 to date – including records of episodes of care, admissions, level of care/treatment and the like.

These requests were lodged on 18 July 2007.  The Department has 30 days to respond. 

This information has been sought in order to provide accurate information to the community to assess the basis of the Department’s claims as to why the Ouse District Hospital is targeted for downgrading.  Tim Morris of the Tasmanian Greens has been instrumental in the preparation and processing of these requests.

If the information is not provided by the deadline, we will take action through the Ombudsman to get it.
